Passport Information
Passport Forms- Please review the passport forms we have available or log onto travel.state.gov to download the applicable form from the PA website that is relevant to meet your travel needs.
The Prothonotary’s office is pleased to offer a full-service passport facility that is able to process a new passport application as well as take photographs for new passport applications and renewal applications.
Items you will need to bring with you to apply for a new passport are:
1) Driver License.
2) Long Form Birth Certificate (This lists the full name of your parents. If you do not have this you will need to order one.) Pennsylvania Records can be ordered at www.health.pa.gov/MyRecords/Certificates or you may visit a vital records office near you and have one processed on site. If you were born in another state visit www.VitalChek.com.
3) Payment for the processing fee for the Prothonotary. We currently accept check, cash or money order.
4) Check or Money order for the application to the U.S. Dept. of State. The State requires every application have its own form of payment.
5) Passport Photographs 2×2 in size. We would be happy to take your photograph here or you may obtain passport photographs from another source.
Birth certificates are sent with the passport application and are returned from the U.S. Department of State to the applicant, although, sometimes in a separate envelope.
There are two different forms of passports:
Passport Book- Is valid for 10 years if the applicant is 16 years of age and older at the time of application and permits travel by any means.
Passport Card- Is valid for 10 years if the applicant is 16 years of age and older at the time of application and is valid only for travel by land and by sea to the following locations: Canada, Mexico, Bermuda and the Caribbean.
If you are age 15 and under at the time of application of a passport book or card it is only valid for 5 years.

Passport Processing Acceptance Agent Fee $ 35.00- per application
Passport Book Adult $ 130.00-U.S. Dept. of State
Passport Book Child $ 80.00- U.S. Dept. of State
Passport Book Adult Expedite $ 190.00- U.S. Dept. of State
Passport Book Child Expedite $ 140.00- U.S. Dept. of State
Passport Card Adult $ 30.00 -U.S. Dept. of State
Passport Card Child $ 15.00- U.S. Dept. of State
** Cannot expedite the passport Card, unless also applying for a passport Book**
The application fee is paid directly to the Department of State. The acceptance fee is paid directly to the acceptance facility. Two separate payments are required at the time of the processing of the passport application.
No appointments are required for passport processing.
